Considering the potential toxicity of this agent, and the patient’s medical history (having been diagnosed with portal hypertension) and the length of the course of the ciprofloxacin this was considered by the panel to have caused a low level of harm. you will need to complete a reflection relating to the incident, the details of which are provided below.

what the outcome could be for the patient of providing their ciprofloxacin, – ensure that there is detailed referenced information in this element, and that you pay full attention to how rapidly irreversible side effects can manitest themselves.
how the situation arose such that you did not consider the patient’s past medical history prior to deciding on the treatment approach and consider how the risk to the patient may be altered due to that history.
why you chose to ignore the specific guidance from January 2024 from the MHRA of “Fluoroquinolone antibiotics: must now only be prescribed when other commonly recommended antibiotics are inappropriate”
ensure you read the entire guidance.
why you did not consider the setting and the ability to monitor effectively if giving an alternative medication which would have been effective in the treatment of the patient.
