Title: Oxygen Error in Emergency Care: A Critical Incident Analysis

Patient A arrived at hospital and was transferred from the ambulance stretcher onto the emergency trolley. At this point Patient A had a non-rebreathe face mask attached to an Oxygen cylinder. Patient A was transferred into resus following an alert call with breathing difficulties.  Oxygen Saturations at this point was 95% on Oxygen.  Once the patient had been transferred across, the doctor in attendance connected the oxygen tubing to the point in the wall. 
The nurse looking after the patient then began the head to toe, A-E assessment, this is when it was noted that the oxygen tubing had been connected to the medical air not the oxygen. The nurse completing the assessment immediately connected the oxygen tubing to the oxygen point.  The patient’s saturations were 95% on oxygen.
An incident form was completed.
