client 3 age 6 :Goal: Improve Focus and Reduce Distractibility
Goal Desc: LAR reports client has trouble staying focused. LAR reports client has a hard time sitting still and gets distract easily.
Obj: Over the next 90 days, client will learn 2-3 new skills to help her improve focus and reduce distractibility.
Int: Skills Training (QMHP) will provide psychoeducation and training in symptom management and recovery skills and how to implement them in their daily living to help him reach previously identified goals.

QMHP met with client to discuss/address (reason for session). QMHP reviewed client actions (homework) from the previous session. QMHP introduced (topic chosen) to address (link to objective). QMHP utilized handouts (cite curriculum or supplemental curriculum) and provided instructions on completing handouts. QMHP provided education about the purpose of handout and reinforced the importance of utilizing skill outside of session to meet previously identified goals. 
Examples of training methods:
QMHP utilized supplemental information about (topic) and used a movie to help reinforce the skill being taught. 
QMHP coached the client when to use the skills learned and reinforced the importance of using this skill to meet previously identified goals.
QMHP modeled skill and allowed the client to practice skill.
QMHP and client role-played various scenarios to allow the client a chance to practice the skills learned. 
QMHP practice and rehearsed skills discussed in session today.
QMHP prompted the client to identify areas of concerns he/she would like to address this day.
Feedback/shaping allowed QMHP to check for understanding of information learned this day. 
Feedback/shaping also allowed the client to identify times he/she could utilize the skill.
