“Appeal for Employment: Overcoming Past Driving Offenses”

I ha d acouple tickets many years ago and because of that it is keeping me from getting a good driving psotion, I have been driving for years and have not had an accident or anything since 2012 but because of the past record (2 tickets) before 2012 it is holding me back. They asked if I would like to appeal their decsion to get a chance at still being employeed with them, here are the questions they are wanting me to answer:
Information concerning the facts or circumstances surrounding the offense(s) or conduct contained herein: 
• Evidence that you have performed the same type of work, post-conviction, with the same or different employers
with no known incidents of criminal conduct, as well as the length and consistency of employment history before
and after the offense(s) or conduct contained in the enclosed background check and/or information shared by
you: 
• Information or evidence of rehabilitation efforts, e.g., education/training, related to the offense(s) or conduct
contained in the enclosed background check and/or information shared by you: 
• Additional employment or character references and other information regarding fitness for the particular position: 
• Information or evidence of being bonded under a federal, state, or local bonding program since the offense(s) or
of having obtained a “certificate of employability” from the applicable jurisdiction:
