Provide the background information on the health disparity and the patient population impacted.
Address the need — what the impact of the disparity is and why improvement is needed.
Discuss the staff you feel would be important to include in this work group.
Describe the PDSA cycle for your project. You will need to be creative in describing the “Study” section of the cycle; this will include what you expect to see in this phase of the cycle.
Make recommendations on next steps of the project.
