Presentation/History
include all relevant SUBJECTIVE history from patient here that supports CC
Physical exam
include all relevant OBJECTIVE data to support CC 
*make sure you place the subjective/objective data in the correct section
**relevant data includes positives/negative to CC
Diagnostics – make sure you provide rationale (why you are doing/differentials) and/or support from CPGs. Sometimes I am unable to discern why you are doing a test related to presentation
Diagnosis (billable)
Rationale – be clear on SPECIFIC symptoms/exam/results from above sections that support diagnosis – don’t just state “symptoms/exam findings” – I want to know that you know what they are
Treatment
should be supported by CPGs or peer reviewed evidence -urls 
primary sources
include page # or heading to review
evidence should be most recent available – caution non US sources for standard of care
include f/u
Rxs include name, strength, dosing, #, refills
