The purpose of an annotated bibliography is to document a list of references along with key information about each one. The detail about the reference is the annotation. Developing this annotated bibliography will create a foundation of knowledge about the selected topic. In your annotated bibliography:
Identify the purpose of the article.
Summarize the information.

Include the conclusions and findings of the article.
Write your annotated bibliography in a paragraph form. The annotated bibliography should be approximately 150 words (1–3 paragraphs) in length.

Make sure the references are listed in alphabetical order, are double-spaced, and use hanging indents.
Summarize what you have learned while developing an annotated bibliography.

Length: 3–5 typed, double-spaced pages, not including the title page and reference page.
Font and font size: Times New Roman, 12 point.

Written communication: Write clearly and logically, with correct use of spelling, grammar, punctuation, and mechanics.
Content: Provide a title page and reference page following APA style.

Create a concept map presentation that shows connections between social determinants of health and nursing practice. Be sure to keep text to a minimum on the slides and provide depth and detail in your speaker notes. Refer to the Supporting Materials section for additional information. An optional template is provided in the What to Submit section. Use at least two current (within the last five years) references to support your work. Specifically, you must address the following rubric criteria: Five Domains: List the five domains of the social determinants of health.

Scenario: A 52-year-old male patient who is a house painter presents to the office reporting chronic fatigue and “mild” chest pain. When he is painting, the chest pain is relieved after taking a break. He reports that the pain usually lasts 5 minutes or less and occasionally spreads to his left arm before subsiding.
