  • Title: Implementing Change in the Workplace: A Comprehensive Plan for Success

    To Prepare:
    Review the Resources and identify one change that you believe is called for in your organization/workplace.
    This may be a change necessary to effectively address one or more of the issues you addressed in the Workplace Environment Assessment you submitted in Module 4 (See attached files for previously completed assessment). It may also be a change in response to something not addressed in your previous efforts. It may be beneficial to discuss your ideas with your organizational leadership and/or colleagues to help identify and vet these ideas.
    Reflect on how you might implement this change and how you might communicate this change to organizational leadership.
    The Assignment (5-6-minute narrated PowerPoint presentation):
    Change Implementation and Management Plan
    Create a 5- or 6-slide narrated PowerPoint that presents a comprehensive plan to implement changes you propose. 
    Your narrated presentation should be 5–6 minutes in length.
    Your Change Implementation and Management Plan should include the following:
    An executive summary of the issues that are currently affecting your organization/workplace (This can include the work you completed in your Workplace Environment Assessment previously submitted, if relevant.)
    A description of the change being proposed
    Justifications for the change, including why addressing it will have a positive impact on your organization/workplace
    Details about the type and scope of the proposed change
    Identification of the stakeholders impacted by the change
    Identification of a change management team (by title/role)
    A plan for communicating the change you propose
    A description of risk mitigation plans you would recommend to address the risks anticipated by the change you propose

  • Title: The Art of Nursing: Navigating Complex Patient Care with Clinical Judgment As a registered nurse, I have encountered numerous situations of complex patient care that have challenged my clinical judgment and required me to use various facets of my professional identity.

    Compose a short reflection of one situation of complex patient care (including clinical judgment) from your own practice (and professional identity) that builds a picture of the multi-faceted concepts supporting what the ‘art’ of nursing means to you.  Focus on the ‘meaningful learning’ you experienced from this situation.

  • Title: “Understanding Klinefelter’s Syndrome: Diagnosis, Implications, and Recommendations for a 14-Year-Old Boy and His Parents”

    Mr. and Mrs. Kim, a couple in their late 50s, are concerned about their son, John, who is 14 years old. John is a tall, outgoing, cheerful-appearing adolescent, who is nevertheless nervous about having his parents accompany him as they seek advice about his possible problems. Mr. and Mrs. Kim have no other children.
    John’s penis and testicles are small for a 14-year-old. His testicles have not matured with puberty, and retain the appearance of those of a child. Mr. Kim says that John has gotten into trouble with school authorities over fighting with other boys in gym class and in the school hallways. This is a new problem during the current school year, but there have been other behavioral difficulties earlier in John’s school career.
    “People make fun of me,” John says. “I’m getting tired of it.” On examination, it is noted that John’s legs are exceptionally long in proportion to his trunk, which is short and stocky. In addition to the small testicles and penis, John has gynecomastia. His facial features are those of a normal 14-year-old. John is scheduled for lab tests to determine his hormone levels and also to have chromosomal testing.
    What is the likely diagnosis for John? What lead you to this diagnosis?
    What recommendations would be made for John and his parents?
    The answer is Klinefelter’s Syndrome.

    Patient is a 58-year-old man who presents to the clinic with complaints of not being able to obtain an erection.  He reports that the symptoms have been worsening and his wife told him to be seen by the provider.  The patient denies any dysuria, fever, or abdominal pain.  He has a history of diabetes type 2 and hypertension.  The patient is currently taking Metformin 500 mg po twice daily and Lisinopril 20 mg po daily.  He reports that he has been having some issues at work and is under a lot of stress.  Upon exam peripheral and femoral pulses are present.  Scrotum is normal.  Normal size testes.  Penis is circumcised.  UA is unremarkable.

    You are an RN working in the Community Outreach Department at Utopia Hospital. You have been asked to give a presentation at the local Women, Infants, and Children (WIC) Nutrition program for a group of 15 expectant mothers and interested partners/spouses/caregivers. The WIC Coordinator has asked you to provide a presentation that will cover relevant health and environmental safety topics the parents should know for the first year of life.
    In this assignment you will create a 8-10 slide PowerPoint for your presentation, with speaker notes. In developing your PowerPoint, take into consideration the health literacy level of your target audience, as well as the demographics of the expectant mothers and interested partners/spouses/caregivers (socioeconomic level, language, culture, and any other relevant characteristic of the caregiver) for which the presentation is tailored.   • Title: Managing Asthma in a Pregnant Patient with Limited Resources: A Case Study

    Your patient is a 23-year-old female. She presents with coughing and wheezing which she stated started about three weeks ago. She is currently 25 weeks pregnant. Her last prenatal visit was one month ago in another state. She has an appointment with the prenatal care provider next week, however her respiratory symptoms brought her to your office today.
    History – Chickenpox as a child. Asthma as a child, diagnosed at age 8 for which she used a SABA when needed. She has not had the need to use an inhaler since she was 19. She takes only her prenatal vitamin. No other acute or chronic problems. She advises you that she is up to date on all immunizations except she has not had a flu shot (it is October).
    Social – Non-smoker, no drug use. She relocated to your state two weeks ago to get away from an abusive domestic situation. She has no support network in this area and has not yet found employment. She has no medical insurance.
    HPA – Non-productive cough x 3 weeks. Wheezing audible from across the room. She states it is like this all day and wakes her from sleep every night. She reports that she is fatigued even in the morning. No other complaints.
    PE/ROS – Pt appears disheveled but clean. Wheezing in all lung fields. T 98, P 82 regular, R 28 no stridor. FH 130 regular. The remainder of the exam is WNL.
    02 98% and FEV 70%
    Directions:
    Construct a narrative document of 4-5 pages (not including cover page or reference page)
    Diagnose the patient based on the above findings and provide your rationale for how you arrived at the diagnosis.
    Develop a treatment plan specifically for this patient, pharmacologic and non-pharmacologic.
    Describe community resources (using your own community) currently available in your state/city to support this patient.
    Provide a communication plan that you will use to ensure the patient is an active participant in the treatment plan. Refer to therapeutic communication concepts.
    Utilize national standards, your pharm and/or patho book, and medical or advanced practice professional sources. Do not use patient-facing sources or general nursing texts.
    Use references to support your concepts. Utilize correct APA formatting (7th edition) and mechanics of professional communication.
