  • Developing a PICO(T) Question PICO(T) Question Development for Effective Database Searching

    When you decide to purchase a new car, you first decide what is important to you. If mileage and dependability are the important factors, you will search for data focused more on these factors and less on color options and sound systems.
    The same holds true when searching for research evidence to guide your clinical inquiry and professional decisions. Developing a formula for an answerable, researchable question that addresses your need will make the search process much more effective. One such formula is the PICO(T) format.
    In this Discussion, you will transform a clinical inquiry into a searchable question in PICO(T) format, so you can search the electronic databases more effectively and efficiently. You will share this PICO(T) question and examine strategies you might use to increase the rigor and effectiveness of a database search on your PICO(T) question.
    To Prepare:
    Review the Resources and identify a clinical issue of interest that can form the basis of a clinical inquiry.
    Review the materials offering guidance on using databases, performing keyword searches, and developing PICO(T) questions provided in the Resources.
    Based on the clinical issue of interest and using keywords related to the clinical issue of interest, search at least two different databases in the Walden Library to identify at least four relevant peer-reviewed articles related to your clinical issue of interest. You should not be using systematic reviews for this assignment, select original research articles.
    Review the Resources for guidance and develop a PICO(T) question of interest to you for further study. It is suggested that an Intervention-type PICOT question be developed as these seem to work best for this course. *Library tip: Walden Library recommends starting your search broadly with one concept or search word and adding more elements one at a time. Depending on your topic, the evidence will not necessarily address all the aspects of your PICO(T) question in one article. Select the most important concepts to search and find the best evidence available, even if that means assembling evidence from multiple articles.

  • Title: Addressing Dual Diagnosis in a 62-Year-Old Man with Substance Dependence and Bipolar Disorder: A Case Study

    After studying Module 3: Lecture Materials & Resources, discuss the following:
    Richard is a 62-year-old single man who says that his substance dependence and his bipolar disorder both emerged in his late teens.  He says that he started to drink to “feel better” when his episodes of depression made it hard for him to interact with his peers.  He also states that alcohol and cocaine are a natural part of his manic episodes.  He also notes that coming off the cocaine and binge drinking contribute to low mood, but he has not responded well to referrals to AA and past inpatient stays have led to only temporary abstinence.  Yet, Richard is now trying to forge a closer relationship with his adult children, and he says he is especially motivated to get a better handle on both his bipolar disorder and his substance use.  He has been more compliant with his mood stabilizing and antidepressant medication, and his psychiatric provider would like his dual diagnoses addressed with psychotherapy.
    What diagnostic information would be most critical to collect in the first visit?
    Which diagnostic measures are recommended by the US clinical guidelines for the treatment of this patient’s family problem?
    Which evidence based harm reduction strategies would you recommend to treat this patient?
    Identify your city. Then refer this patient to three agencies near you that would support positive health outcomes for this patient.  (These agencies must not have been used in past discussion posts). What was your rationale for choosing these three agencies?

    THE ASSIGNMENT
    Step 1: Choose one of the following vulnerable patients to create a Medication Guide for the patient:
    Patient 1: 26-year-old female with a diagnosis of major depressive disorder and social anxiety disorder who is increasing in isolation and poor self-care. She is in her third trimester of pregnancy.
    Patient 2: 16-year-old male with a diagnosis of major depressive disorder, severe. He has seen a therapist weekly for the past 6 months and has had minimal change in symptoms. He has expressed thoughts of wanting to die.
    Patient 3: 72-year-old male with diagnosis of major depressive disorder and panic disorder. He has cardiac history and takes antihypertensive medications.
    Patient 4: 8-year-old Asian female with a diagnosis of severe depressive disorder presents to the office with a report of worsening symptoms. She has never taken psychotropic medication before.
    Step 2: Create a Medication Guide for a patient. In your guide, you should provide the following specific instructions for the patient:
    Describe the chosen classification of medications, from the classification category, for your chosen vulnerable patient. Explain your rationale for your choice.
    Explain what dose you would start the chosen medication with and the frequency.
    Discuss how the medication works to treat their symptoms.
    Explain how long they should take the medication.
    Discuss the typical or common side effects of the medication.
    Explain the urgent or emergent considerations for the patient taking the medication.
    The Medication Guide should also include:
