For this assignment, you will develop a realistic clinical case on a topic that is of interest to   you. 
Content Requirements
You will create a realistic case study and include appropriate and pertinent clinical information that will be covering the following:
Subjective data: Chief Complaint; History of the Present Illness (HPI)/ Demographics; History of the Present Illness (HPI) that includes the presenting problem and the 8     dimensions of the problem; Review of Systems (ROS)
Objective data: Medications; Allergies; Past medical history; Family history; Past        surgical history; Social history; Labs; Vital signs; Physical exam.
Assessment: Primary Diagnosis; Differential diagnosis
Plan: Diagnostic testing; Pharmacologic treatment plan; Non-pharmacologic             treatment plan; Anticipatory guidance (primary prevention strategies); Follow up    plan.
Other: Incorporation of current clinical guidelines; Integration of research articles;    Role of the Nurse practitioner
Submission Instructions:
It must be an original work and logically organized, formatted, and cited in the          current APA style, including citation of references.
Incorporate a minimum of 5 current (published within the last five years) scholarly    journal articles or primary legal sources (statutes, court opinions) within your work.  Journal articles and books should be referenced according to APA style (the library has a copy of the APA Manual).

Op-Ed Assignment criteria:
-Like a Magazine. Casual, exciting to read, not traditional academic.

– Pick a topic on health informatics aka -cyber security, AI (generative, machine learning). Different ways informatics is used in health care like virtual care or social media (Digital technology). Electronic health record (Interoperability, platforms for multiple care settings… as a user, what data collected?). Social issues, politics.. Often done for advocacy for your position and gain attention from the public. 
– Pick a health care problem/issue from the lens of health informatics. 
– Can look at it from a specific perspective like EHR or general like health media/how getting info across over media (example like podcast usage). 
– Take a stance and reflect your views accordingly. 
–  Write a short article in the style of editorial, more conversational, still provide references and cite it (in text and at the end). 
– Shouldn’t look like a traditional academic paper 
– Add a byline at the end— Sonam Prasad, Masters in nursing student, working as a critical care RN, interested in/passionate about/advocate for…  (I can also just edit this part accordingly to personalize it to myself). 
– Examples included in outline

o    Research articles that relate to legislation that has affected current healthcare policies such as: Health Insurance Portability & Accountability Act (HIPPA), Affordable Care Act (ACA), Mental Health Parity Act (MHPA), Emergency Medical Treatment and Labor Act (EMTALA), Paperwork Reduction Act
Develop a PPT presentation with at least 20 slides (1 with APA references) explaining the health policy and policy-making process. Discuss the roles nursing organizations have played in influencing policy as it relates to the health policy/legislation selected. Discuss how this policy has impacted the healthcare system. Include how nurses can become involved in policymaking.

Choose one primary research (1) article used in the literature review—less than five years or if you prefer in the EBP project. It is preferable to use a randomized controlled trial or the highest level of evidence. Attach a copy of the article.
Write a 1-2 page critique on the article (excluding cover and reference page).

How realistic is it? How does the method address questions of validity? (independent, dependent variables).
How does the researcher overcome the limitations of the method?
Are there large limitations or minor ones?
How will these limitations affect your ability to use this data to answer your research question?
Describe the results by answering:
How were the results interpreted?
How were they related to the original problem (author’s view of evidence rather than objective findings)?
Look at DISCUSSION are the author(s) able to answer the question (test the hypothesis) raised?
Did the research provide new factual information, a new understanding of a phenomenon in the field, a new research technique?
How was the significance of the work described?
Did the reported observations/interpretations support or refute observations or interpretations made by other researchers?
Did the researcher report limitations.
Describe the significance of the research by answering:
Did the research make a significant contribution to human knowledge?
Did the research produce any practical applications?
What are the nursing implications of this research?
How do you evaluate the significance of the research?
Papers will be graded by rubric. Please take time to review the rubric so that you are aware of the expectations for the research critique paper.   
Grading Rubric:
– The journal’s primary research article is current, peer-reviewed, contains the author’s name(s), year of publication, and corresponds to the list of references.
– The summary clearly outlines the main research question, methods, results, and implications in the student’s own words.
– Identifies the rationale for the study’s aim/ purpose by clearly identifying a gap in the literature.
– The study design is described in detail. If quantitative data were collected, psychometric properties of instruments are provided (e.g. validity and reliability) are provided. The sampling strategy(inclusion/ exclusion criteria), study setting, and data collection procedures are described in detail. The approach used for data analyses is described.
– Results/products/outcomes are described in detail and alignwith description ofstudy methods. Analyses ofdata are sophisticated and precise
– Stated recommendations are based on a thorough examination of evidence, a clear explanation of reasonable alternatives, and an evaluation of possible consequences. Conclusions and recommendations are congruent with the strength of evidence reviewed, and an appropriate interpretation of both clinical and statistical significance of the findings.
– The student provides an insightful analysis of the article, answering all or nearly all of the questions assigned. The student articulates novel ideas that clearly go beyond what is in the article itself. The analysis is clear and rational.

J.S. is a 42-year-old man who lives in the Midwest and is highly allergic to dust and pollen and has a history of mild asthma. Investigate the condition of asthma and the manifestations of the disease. Analyze the case study provided and determine what symptoms support the diagnosis of asthma.

I work as RN in Mount Sinai Medical Center, Miami Beach, FL in a Cardiac Telemetry/Step Down Unit.
Clinical problem could be focus on patient education at the discharge because patient sometimes don’t understand their illness well and how to keep the medications adherence and don’t follow up medical appointments.
Therefore, if patient educations is not well explained by the staff or nurse how make the discharge in this case the patient doesn’t have a good understanding and there are high probabilities for the patient to return to the hospital due to non-medication compliance, diet, complications, adverse effects etc.
Now to address this issue the hospital is having a well-trained nurse to focus on patient discharge to improve patient education and quality improvement.
