The examination will consist of short answer conceptual questions and practical exercises/problem questions. The questions will be comparable to those covered in class; therefore, the best form of revision will be to work through lecture and tutorial questions, exercises and problems.

For your final paper in this class, students will identify an issue of social justice/injustice, a current “hot button” issue or policy impacting education. Students will conduct research on the background and context of the chosen issue, analyze the broader systemic view (e.g., national, state, and /or local), interview 3 individuals (including multiple perspectives from different constituencies) and discuss possible solutions for change (drawing on the Design Justice Principles and students/communities’ Funds of Knowledge). Pulling in research and interests from prior assignments is welcomed and encouraged. The paper should be no more than 4 pages—what is important is that you cover each of the required sections in the rubric.

Use the article from the Galen Library database that you selected for your Week 5 Discussion. Complete the following steps in a paper using Microsoft Word:
Begin with an APA-formatted title page (be sure to use the 7th edition APA template Download 7th edition APA templateOpen this document with ReadSpeaker docReader ). Insert a page break after the title page to begin your summary.
Beginning on page 2, write a 500-word article summary that includes a topic sentence.
An academic summary focuses on identifying what is most important in the article and relating those facts in APA format. You can use your Academic Writer account for help with APA format.
The topic sentence should tell readers the title of the article, the name of the author, and what you’ve determined the main point of the article to be. This paper should be an attempt at formal, academic writing using APA style. View the following sample paper to guide your APA formatting: Sample APA Style Paper (available in Canvas).
Insert a page break to create your References page. Since you’re going to be summarizing an article that has been published in an academic journal and you’re going to have found that journal in one of Galen Library’s online databases, the reference on the References page will be written as follows:

Items of note:
For the volume and issue number, you will only list the numbers.

In this task, you will apply strategies for integrated delivery systems by examining current and future challenges and opportunities of integrated systems. You will select an existing healthcare organization from any one of the lists provided in the web links section. After researching this organization, you will propose an integrated delivery system that incorporates a health plan, a hospital or group of hospitals, a skilled nursing home, and an innovative healthcare delivery model (e.g., accountable care organization, patient-centered medical home model). Your proposed system must meet the rising health demands of a large population of individuals 65 or over and the regional needs of the healthcare organization. Your proposal should also achieve the goals of integrated systems and person-centered care.
Note: Each of the course assignments should be completed in full before attempting this task. The two course assignments are submitted as Task 1.

REQUIREMENTS
Your submission must be your original work. No more than a combined total of 30% of the submission and no more than a 10% match to any one individual source can be directly quoted or closely paraphrased from sources, even if cited correctly.
