Develop a 2-4 page preliminary care coordination plan for a selected health care problem. Include physical, psychosocial, and cultural considerations for this health care problem. Identify and list available community resources for a safe and effective continuum of care.
Introduction
The first step in any effective project is planning. This assessment provides an opportunity for you to strengthen your understanding of how to plan and negotiate the coordination of care for a particular health care problem. Include physical, psychosocial, and cultural considerations for this health care problem. Identify and list available community resources for a safe and effective continuum of care.
NOTE: You are required to complete this assessment before Assessment 4.
Preparation
As you begin to prepare this assessment, you are encouraged to complete the Care Coordination Planning activity. Completion of this will provide useful practice, particularly for those of you who do not have care coordination experience in community settings. The information gained from completing this activity will help you succeed with the assessment. Completing formatives is also a way to demonstrate engagement.
Scenario
Imagine that you are a staff nurse in a community care center. Your facility has always had a dedicated case management staff that coordinated the patient plan of care, but recently, there were budget cuts and the case management staff has been relocated to the inpatient setting. Care coordination is essential to the success of effectively managing patients in the community setting, so you have been asked by your nurse manager to take on the role of care coordination. You are a bit unsure of the process, but you know you will do a good job because, as a nurse, you are familiar with difficult tasks. As you take on this expanded role, you will need to plan effectively in addressing the specific health concerns of community residents.
To prepare for this assessment, you may wish to:
Review the assessment instructions and scoring guide to ensure that you understand the work you will be asked to complete.
Allow plenty of time to plan your chosen health care concern.
Instructions
Note: You are required to complete this assessment before Assessment 4.
Develop the Preliminary Care Coordination Plan
Complete the following:
Select one of the health concerns in the Assessment 01 Supplement: Preliminary Care Coordination Plan [PDF] resource as the focus of your care coordination plan. In your plan, please include physical, psychosocial, and cultural needs.
Identify available community resources for a safe and effective continuum of care.
Document Format and Length
Your preliminary plan should be an APA scholarly paper, 2–4 pages in length.
Remember to use active voice, this means being direct and writing concisely; as opposed to passive voice, which means writing with a tendency to wordiness.
In your paper include possible community resources that can be used.
Be sure to review the scoring guide to make sure all criteria are addressed in your paper.
Study the subtle differences between basic, proficient, and distinguished.
Supporting Evidence
Cite at least two credible sources from peer-reviewed journals or professional industry publications that support your preliminary plan.
Grading Requirements
The requirements, outlined below, correspond to the grading criteria in the Preliminary Care Coordination Plan Scoring Guide, so be sure to address each point. Read the performance-level descriptions for each criterion to see how your work will be assessed.
Analyze your selected health concern and the associated best practices for health improvement.
Cite supporting evidence for best practices.
Consider underlying assumptions and points of uncertainty in your analysis.
Describe specific goals that should be established to address the health care problem.
Identify available community resources for a safe and effective continuum of care.
Organize content so ideas flow logically with smooth transitions; contains few errors in grammar/punctuation, word choice, and spelling.
Apply APA formatting to in-text citations and references, exhibiting nearly flawless adherence to APA format.
Write with a specific purpose with your patient in mind.
Adhere to scholarly and disciplinary writing standards and current APA formatting requirements.
Additional Requirements
Before submitting your assessment, proofread your preliminary care coordination plan and community resources list to minimize errors that could distract readers and make it more difficult for them to focus on the substance of your plan. Be sure to submit both documents.
Portfolio Prompt: Save your presentation to your ePortfolio.
Course Competencies
By successfully completing this assessment, you will demonstrate your proficiency in the following course competencies and scoring guide criteria:
Competency 1: Adapt care based on patient-centered and person-focused factors.
Analyze a health concern and the associated best practices for health improvement.
Competency 2: Collaborate with patients and family to achieve desired outcomes.

“Controversy and Consequences: Examining the Supreme Court’s Ruling on Abortion Rights in America” “The Importance of Citing Sources in Academic Writing” Introduction Academic writing is a critical component of higher education, and it requires students to engage with various sources to support their arguments and ideas. However, it is not enough to simply use
This is the assignments content.
Abortion has long been one of the most reliably contentious issues in American society. For five decades, a woman’s right to access an abortion has been upheld via Supreme Court ruling in the 1973 case of Roe V. Wade. Since that time, access to abortion has been reliably upheld time and again, year after year, by justices from both the “left” and the “right”, and despite the desire by some more conservative groups to overturn that decision, had largely been considered “settled” and “precedential”, as newly appointed Supreme Court justice Ketanji Brown Jackson stated during her nomination process. The reason for this assumption is because the U.S. legal system is built up the concept of precedent, or “stare decisis”, a Latin term which means to “stand on decided cases”. What this means is that U.S. courts, including the Supreme Court, is beholden to consider not only previous decision by the court, but also the length of time, and the number/veracity of those challenges over the years. As previously mentioned, Roe had been reaffirmed time and again by both liberal and conservative justices, but we now know that the court, driven largely by the new conservative majority within, has rendered an opinion that goes against previous precedent by citing that the original decision was arrived at via poor legal reasoning, and which has effectively overturned federal guarantees of the right to access an abortion at will.
The reasoning given for this massive shift is that abortion was not part of the American legal fabric created by the constitution, but rather by improper interpretations of the rights conferred by the constitution at the time that the case was first heard. It should be noted that the Supreme Court ruling does not mean that all access will be denied, it only means that individual states will be allowed to determine how wide to grant access and under what conditions. For some women in more liberal states, they will likely find themselves less affected personally, while women living in states like Texas, Oklahoma, Florida, etc. will see their access significantly curbed to the point where some would consider it to be nonexistent since the restrictions put into place often require the services to be sought out before a woman may even know that she’s pregnant, and also by not providing for exceptions on issues of rape, incest, or other medical conditions which would result in a nonviable birth but which may not be life-threatening to the woman. This landmark opinion by the court also reverberates beyond simply the issue of abortion itself, given that it was overturned on the basis that there is no tradition of this practice being part of the fabric of American life at the time the Constitution was written. What do you think about this issue and the court’s ruling that overturns Roe v. Wade, do you agree or disagree with it, and why?

“Amending and Correcting a Thesis on the Correlation between Sales and Supply Chain Management in Saudi Arabian Manufacturing Companies”
Hello buddy, Good day, I would want you to amend and correct my whole thesis where necessary with the following points to work on to be addressed in the thesis-
1) Delete sabic and aramco where necessary in the whole thesis because we didn’t use them in our data and results. Instead talk about these as regards to the hypothesis- A) There is a positive correlation or relationship between sales and supply chain management in manufacturing companies in saudi-arabia. Sales also has a unidirectional association with supply chain management.
B) Return on Asset also positive with supply chain management and unidirectional association with supply chain management too. C) Return on Equity negative with supply chain management in manufacturing companies in saudi. With unidirectional association between them as well. D) Cash Conversion Cycle has a negative correlation with supply chain management too. But bidirectional association with supply chain management.
E) you can also put more hypothesis apart from the one listed above that is related to the research, please only related work. 2) You should also state the year 2012 to 2022 is the time series of the data collection for our research. 3) You should remove the primary data in the research methodology, where it is stated in the whole of the thesis. 4) from the supply chain management PDF shared as an attachment, I would want you to draw discussion from the attachment and explain it in the literature review this variables which are- Return on asset, Return on equity, Cash Conversion Cycle, Sales and Supply Chain Management Ratio. 5) You can mention in the thesis the reason why we are doing the test for supply chain management in Saudi Arabia for manufacturing companies is because no one has ever done what we did in the past, no one did the test for co-integration test, casualty test and this is the first time we are using such test for supply chain management in Saudi Arabia for manufacturing companies.
6) you can also put this in the statement of purpose or significance of study or statement of problem or introduction that no one has ever done it before and there is a study gap that needs to be filled.
7) After you finish stating or talking the above points as requested, you can state or talk everything of the points in the abstract likewise also. Remove Return on investment in the abstract because I do not need it in the thesis as part of the variable, put about what i discussed in the variable of our research in the abstract.
8) Do not forget to mention why did we choose saudi manufacturing companies, the vision that they have to become the best supply chain management in the world from other countries. We use saudi-arabia because they have future visions. There is no too much paper that talk about saudi arabia in supply chain management and not too much people has talked about it and that is why we are talking about it in our thesis.
9) Do not forget to include more references in the reference section of our thesis because the references in the thesis are little and more references needs to be added as in relation with the work of the thesis.
This is all for now. Thanks for your ùnderstanding.
Anticipating your positive response. Best Regards. -

“Exploring Solution-Focused and Narrative Models in Substance Abuse Treatment: A Student-Led Discussion Post Group Project” Introduction In the field of substance abuse treatment, there are numerous theories and models that have been developed to address the complex issues surrounding addiction
Part 1 – This portion is formal, and must be written using APA 7th ed style and formatting.
This is a formal paper regarding your professional experience of the Student Led Discussion Post Group Project. You are required to provide the following information:
A history of the theorist and theory. Each student should write about one of the theories (for example if you are in in week 7, one person should focus on Solution-focused and the other on the Narrative Model).
Part 2 – Does not require APA 7th ed, although if needed feel free to discuss
Provide insight into the way in which the group composed the use of the theory in the discussion post; the reason for wanting to use an article, the question for the class, etc. What was the significance of leading the discussion to help understand the theory model regarding its approach to substance abuse
What did you like and what would you have changed as a group?
Remember, this is a group written assignment, so consider splitting up the work, and submit it your first part and the second part in your Turnitin respectively. For example, Zack is working on Solution-Focused and Amy is working on Narrative. Zack will submit his section on Solution Focused and Part 2 to Turnitin , Amy will do the same for Narrative respectively.
